The Advanced Practice Registered Nurse is responsible for diagnosing and treating a variety of diseases and injuries in a Family Practice model setting; provides comprehensive, outpatient medical services for individuals and families on a continuing basis. Responsible for Transitional Care Management for patients recently discharged from hospital or emergency department and bridge the care until patient establishes with their primary care physician. Responsible for conducting Annual Wellness Visits conducted in conjunction with CMS guidelines for Medicare and Medicare Advantage patients.

Graduate of an accredited Advanced Practice Registered Nurse program; certified by the Nevada State Board of Nursing as an Advanced Practitioner of Nursing. Must be licensed in the State of Nevada.
